AI, Addiction Treatment, and Ethics: Innovation at a Crossroads in the UK

Back in February 2026, the UK government put £20 million behind its Addiction Healthcare Goals programme. They want to shake up drug and alcohol treatment - a big promise, really - by bringing in AI, mobile apps, wearables, and virtual care. The goal? Cut harm, save lives, and help more people build a life in recovery.
It’s a big moment. Addiction services in the UK have struggled for years - never enough money, plenty of stigma. Now, with this cash, the sector becomes a testing ground for tech. But don’t be fooled into thinking you can just throw AI at the problem and walk away. The real challenge is figuring out how you use the technology, who gets to be in charge, and how you make sure it actually supports people, not just numbers on a screen. Because recovery isn’t just about data or algorithms - it’s built on trust, empathy, and real human connection. Technology should lift up the people doing the work, not push them aside.
So, what does AI actually do here?
Innovate UK is leading the charge, and the developers are busy designing tools that can:
- Spot when someone’s at risk of relapse
- Tailor support to each person’s habits and needs
- Let clinicians keep an eye on people remotely
- Make treatment easier to access, not just stuck inside a clinic
There’s a lot of hope around this. But let's be real: addiction care works with some of the most vulnerable people you’ll meet. AI should help clinicians make better decisions, never try to replace them.
But what about the tough questions - ethics?
Privacy’s the first hurdle. People share deeply personal stuff: where they use drugs, their mental health battles, trauma, times they’ve relapsed. AI feeds on lots of data, which means:
- What if there’s a leak or someone abuses the info?
- Who actually owns it?
- And when does tracking become surveillance, not support?
Even a tiny breach can wreck lives. Laws help (like the new Data (Use and Access) Act 2025), but the system is still delicate.
Then there’s bias. AI can only learn from the data you give it. If it’s badly designed:
- People on the margins - minorities, folks with little money - get left out
- It makes bad predictions or offers useless advice
- It actually widens healthcare gaps
Picture this: an app that knows how city clinics work, but completely misses the mark in rural towns.
We can’t forget about autonomy, either. Tech should help people move forward in recovery, not turn them into passengers. Relying too much on AI can nudge people a little too hard, maybe even kill their drive to make their own decisions.
And let’s face it - AI’s got no empathy. People recover best when they build real relationships. If we swap out conversation for tech, it gets pretty lonely.
Who answers for mistakes? Algorithms get things wrong. Someone has to take responsibility - the app maker, the doctor, or the clinic? If no one can explain how the system made a decision, that’s a real problem, especially when lives are on the line.
There’s another trap: automation overload. Clinicians could end up depending too much on AI and lose the judgment and thinking that make them good at their jobs. Tech should help, not take over.
Transparency matters. Patients need to know what’s happening - where their data’s going, if AI is involved, and what that means for them. Can they say no? Do they really understand what’s at stake?
And who creates all this powerful new tech? A lot of it comes from private companies - chasing profits and patients at the same time. That opens up a lot of cans of worms:
- Apps and ads designed to keep people clicking, not necessarily getting better
- Over-the-top claims promising miracles
- Targeting people during their lowest moments, when they’re most vulnerable
- Expensive subscription fees that shut out those who can’t pay
- Patient data being sold or traded, sometimes without real consent - even if it’s “anonymised”
There’s a pushback, though. EMCAT (the Ethical Marketing Campaign for Addiction Treatment) are out there, calling out shady marketing moves and outright exploitation - like patient brokers shuffling people between clinics for a fee. EMCAT’s code bans this stuff and pushes for actual honesty. Regulators are stepping up, too. The Advertising Standards Authority has started cracking down. Some ad platforms now make people come clean if they’re middlemen, not real clinics.
But here’s another hurdle: the digital divide. All these high-tech solutions need gadgets, internet, and some confidence online. People already on the edges - maybe homeless, maybe without internet - risk being left behind.
And while prediction sounds good, it’s tricky. AI can flag who’s likely to relapse, but…
- Should we stick people with a “high risk” label?
- What if those predictions make things worse?
- How do we actually use that information without causing harm?
Regulators do matter here - the Care Quality Commission, General Medical Council, the Advertising Standards Authority. And new rules like the EU AI Act and guidance in the UK try to keep things fair, open, and safe for patients. But it’s not exactly easy changing old habits.
So, what’s the takeaway?
The new £20 million is a big deal. AI can help spot relapse, make treatment more personal, and reach more people who need help. But if this is just about replacing real human care with machines, it’s not going to work.
People always need to stay in charge. Patients should know exactly what’s happening with their data and what the AI’s doing in their care. Someone needs to watch for bias, harm, and dishonest advertising - every day, not just once. It’s not just about fixing algorithms but also about keeping the marketing honest.
Technology can help people heal, find support, and regain control of their lives - if we use it wisely. Mismanaged, it just adds to the struggles they already face. Let’s use it right.

NHS Fife coordinates the entire programme, overseeing progress, reporting, and steering funded projects through feasibility and demonstration phases. Most projects receive up to £100,000 for feasibility studies, with one project receiving up to £500,000 for extended real-world testing. This ensures that regional pilot implementations, such as hospital or community care trials, are monitored and guided carefully.
